Multipurpose Ultrasonic Tips! By Integrated Endodontics

Endodontic therapy includes removal of tooth structure to reach the apex. Different cutting instrument like airrotor or slow speed diamond cutting instruments are used during endotherapy. All of us would accept that we do a lot of tooth destruction while entering the pulp chamber & while locating the canal orifice. In case of calcification in the chamber, use of airrotor has chances of perforation at the furcal area or floor of the chamber. Ultrasonic tips have emerged as an effective and conservative approach to high-speed instruments. Integrated Endodontics – India developed some of the effective useful instruments (i-SuperTip) for day to day practice. Since action of piezo tips is linear in action, only piezo endo tips and piezo scaler machines are recommended in endodontics.

 

High-speed Rotary Instruments Are Replaced By Ultrasonic Tips For The Following Reasons:

Method 1:

Removing the pulp stones in the chamber or locating the canal orifice like MB2, D2. Use i–SuperTip # 30 or # 80 at power 10% to 30%.

Please note – Maintain long axis alignment all the time while using Ultrasonics. Action – Peking / Brushing motion (Be gentle). (# 30 for thin canals & # 80 for large canals.) One can improve straight-line access with these tips.

 

Method 2:

To agitate warm 3% NaOCl & liquid 17% EDTA during cleaning and shaping. Use I–SuperTip passively for 60 seconds. Use power 20% to 50% while agitating – the tip should not bind to the walls. Act passively in & out.

Please note – The file should not touch any of the walls and i–SuperTip should be 4mm short of W.L. all the time.

 

Method 3:

During warm obturation – complete the obturation with cold lateral compaction technique. Sear it off at the canal orifice. Then pick up i–SuperTips # 30 or # 80 and activate at power 20% to 50% for 30 sec. Once the tip is heated, transfer the heat to the mass of GP.

Once the GP is heated up to 4mm by heated i–SuperTip uses a 5% flare spreader to combat GP against the canal wall up to 4mm. Then repeat the procedure by taking heated i–i-SuperTip in a created space and gently transferring the heat from i–I-SuperTip to GP mass. Take a 5% flare spreader and compact the GP close to working length (W.L) add one accessory GP cone in the created space.

If required repeat the procedure and add more cones. Confirm the 3 dimensional obturation on radiograph. Use an ultrasonic tip without water to warm the GP master cone. Since GP is a heat conductor, heated I–SuperTip should be taken 4mm short of W.L.

Please note –Heated tip should not compact or condense the G.P. It is just taken closer to G.P. mass to transfer the heat and the flare 5% spreader will displace the G.P. to create warm lateral compaction.
